      How is Bichectomy Application Performed?

      Bichectomy is also known as “cheek thinning”. Bichectomy can be applied by following the steps below;

      • Local anesthesia is first applied to the patient.
      • Mouth cleaning should be done thoroughly. Oral care before bichectomy is very important to prevent possible infections.
      • Small incisions are made near the fat bags on the cheeks.
      • Fat tissue is taken from the opened incisions.
      • Opened incisions are closed by suturing.
      • Afterward, the application is completed with a compressive bandage.

      Bichectomy application takes approximately 30 minutes to 45 minutes.

      • The patient can be discharged on the same day.

      What is the recovery
      period for bichectomy?

      As in every aesthetic application, the healing process in bichectomy may differ from patient to patient. However, if the situation is to be generalized, the healing process begins within two weeks of bichectomy. The care of the person during the healing process also depends on paying attention to the foods they eat. After the bichectomy, the person should consume soft foods and consume cold drinks. Thus, there is no bleeding. The other thing to be careful about is not to be exposed to too much sun. If inflammation occurs after the operation, this inflammation will heal in three to six months. Inflammation usually occurs in the chin area. Bichectomy aesthetic application is applied to make the cheekbones on the face more prominent and to make the face appear longer.

      Is bichectomy surgery permanent?

      It is the removal of a part of the fat pads, which we call the bichat fat pads, located in the vicinity of the blowing muscle on both cheeks, which is removed in the bichectomy surgery, also called the Hollywood cheek surgery. This procedure can be performed under local anesthesia or under general anesthesia. The procedure takes about 30-45 minutes and is a permanent procedure. After the procedure, edema develops on the cheeks for a few days, which may last 1-2 weeks. Even after the edema has passed, the desired effect has not yet occurred. Between 3-6 months, the lower soft part of cheek which is immidiately under the cheekbones thins a little, making the cheekbones appear more prominent. With just bichectomy surgery, facial thinning cannot always be achieved as desired. Because it is not only bichat oil that provides the width of the face; There are many factors such as salivary glands, masseter muscle, patient’s skin thickness and subcutaneous adipose tissue, maxilla bone mass. By removing only the cheek part of the fat pad that extends to the temples, there may not always be a very noticeable thinning of the face.

      When does the bichectomy swelling go away?

      After the bichectomy surgery, there is edema on the face for 1 week. If this edema is unilateral and painful, it should not be thought of as swelling after every bichectomy operation, and you should definitely consult your doctor. I would like to specifically mention 2 complications that can occur after bichectomy surgery. One is a hematoma (blood pooling) and the other is an abscess. Hematoma is an event that results in bleeding in the surgical field due to vascular trauma while surgery, but due to the stitches, this blood accumulates on the cheek instead of flowing out, resulting in swelling of the cheek. After every bichectomy operation, there may be slight mucosal bleeding, but these are not a problem. The solution is to open the stitches again, find the bleeding source and control the bleeding. In my own practice, to my patients; After bichectomy, I wrap the entire head for 1 day in order to minimize the possibility of bleeding, putting pressure on the cheeks. Nutrition after surgery is also important in this surgery. We generally recommend patients to be fed transparently for 2 days after the surgery. In other words, we recommend that they be fed little but often with smooth, transparent foods close to water. These include compote, fruit juice, broth (not hot), milk. The reason for this is that the grains may damage the surgical sutures and reach the operated area from the operation sites. This results in the formation of the second complication I mentioned, abscess. Unilateral significant asymmetric swelling, pain and fever may bring abscess to mind. If such feared situations do not occur, the swelling starts to decrease after the 2nd week and 3.4 . In months, the face gets a little thinner in the lower part of the cheekbones.

      How much fat is removed in bichectomy surgery?

      The fat removed in bichectomy surgery is the cheek part of the bichat fat pack . We do not remove the part that extends to the temples. This may vary in individuals. In my own practice , I extract different amount ıf fats in every individuals , it changing from 2 cc to 3.5 cc of fat from a single side . The surgery can be done with small incisions made in the mouth under local anesthesia, or these fats can be removed during facelift surgery. However, instead of removing it, I replace it during facelift surgery and use it for augmentation.

      Can Removed Fats be Injected to Any Area of ?

      Bichectomy is an oral surgery. A 1 cm incision made above or below the place where the salivary gland opens into the mouth is usually sufficient for this. However, when the fats are removed from the mouth, they are contaminated with saliva. There are billions of beneficial flora bacteria in the mouth. However, if these bacteria pass from the oral cavity to the blood or body cavities, they become our enemies. Our face is one of the places where fat injection is done the most in the body. We inject fat from other parts of the body in order to fill under our facial skin and subcutaneous tissue, which loses volume as we age, to gain volume and to benefit from the rejuvination effect. We need to pay close attention to sterility in this process. In general, these areas can be the abdomen, hips or arms, but we do not use the fats from bichats contaminated with the mouth. Because no one wants to take the risk of infection by injecting contaminated fats into any face with millions of bacteria on them.

      Where is the bichectomy procedure performed and with which type of anesthesia?

      Bichectomy surgery takes about 30-40 minutes. We do not experience any pain during the procedure thanks to a few cc of local anesthetic. Therefore, I perform the application under local anesthesia in my own clinic. However, it can be done under general anesthesia when diseases such as panic attacks and asthma are accompanied. When combined with other surgeries, it can be applied in the hospital under general anesthesia.

      Is there any pain felt during the operation?

      Thanks to the local anesthetic agents applied to the cheeks of the patient before the bichectomy surgery, patients do not feel pain during the operation. The procedure is extremely comfortable in the patient sitting position. Thanks to the intramuscular injection of painkillers after the procedure, the patient does not have much pain for 7-8 hours (this is not necessary, oral painkillers can also be given), the patient can overcome the next process with oral painkillers. On the 2nd day, there is not much pain without taking painkillers.

      Does bichectomy surgery restrict eating and drinking?

      After the bichectomy surgery, in order to prevent the escaping of the food into the gap which is formed after the removing the fat , and causing infection there. We recommend that patients be fed with smooth, liquid foods such as compote, fruit juice, broth, milk for 2 days, and rinse their nets with mouthwash after each feeding. In the next 2-3 days, soup and soft foods are started, and then normal nutrition can be started 5 days after surgery . Otherwise, complications such as infection and abscess may develop. If an abscess develops, it should be drained immediately, and broad-spectrum antibiotics should be used.

      Is it necessary to stay in the hospital after the procedure?

      Bichectomy surgery is a day procedure. No hospitalization is required. After the operation, the patient can be observed in the clinic for 2-3 hours and then discharged. If the procedure is performed only under general anesthesia, a minimum of 4 hours is observed, and after 4 hours, the patient is fed with liquid foods. Afterwards, if the patient’s vital functions and urine output are normal, the patient can be discharged home.

      Is there an age limit for bichectomy surgery?

      There is no age limit for bichectomy surgery. However, a 10- or 15-year-old individual never goes to a specialist and requests bichectomy surgery. Even if there is such a thing, no physician; Since the individual does not yet have the ability to decide on a change about his own body, he does not do such a procedure. Legally, this ability is acquired at the age of 18 in our country, but in my own practice, I find this process more appropriate for the 23-24 age group and above, where the person has completed the stages of accepting his/her own body.

      Is it necessary to stay in the hospital after the procedure?

      After the bichectomy operation, patients can return home on the same day, can drive, There is no very disturbing pain, They can do their daily work. It is not necessary to stay in the hospital. There may sometimes be temporary facial nerve paralysis after the operation, but it has been reported that it passes within a few hours, very rarely 1 month. If complications such as hematoma, abscess, and infection do not occur, swelling usually decreases significantly in 1 week. In my practice, on the first day, I am dressing to apply pressure to the cheeks. After 1 day, I want to remove this dressing and check if there is any problem. Then I see Week 1.
