Prominent ear aesthetic surgery is a procedure that includes the surgical replacement of the deficiencies of some structures in the anatomy of the auricle or the reduction of the excess, or both. Prominent ear deformity is most commonly caused by the complete or partial absence of the antihelix fold in the normal auricle anatomy and the excessive volume of the structure we call the concha. This fold, which is not in the process, is created with suture techniques or a little bit is removed from the structure we call the turbinate, or both processes are applied at the same time. The operation begins with the skin incision made behind the auricle and the removal of some excess skin, and the operation takes approximately 2 hours, 45-60 minutes for each ear. In my own clinic, I perform this surgery under local anesthesia or general anesthesia in children at the hospital.

It’s a question that I get frequently. In children, the auricle completes its development at 5-6 years of age. Therefore, it can be done after the age of 6. So why is early surgery preferred at this age? Because the problem that creates this aesthetic disorder also causes psychological disorders in the child, especially a child who is in his school age. Is Prominent Ear Surgery
Possible Without an Incision?
Yes, this surgery is possible without any surgical incision. Generally, in children and some adults, when the auricle has not hardened yet and the structure we call the turbinate is not abnormally bulky, the sutures can be removed with some techniques and an antihelix fold can be formed without making an incision. This procedure, which is performed only by suturing, can be performed in suitable cases, but the risk of recurrence is slightly higher than the surgical procedure.

What is The Process After Prominent Ear Aesthetic Surgery?
After I do this surgery, I dress and wrap the ear and 2 days later, I open it and see it myself. If I performed the operation under local anesthesia, I do not hospitalize the patient, but if I performed it under general anesthesia, I can hospitalize the patient for a day. After 2 days, I can dress it for one more day or switch to ear bandages, depending on the situation. I have my patients use ear bandages every hour of the day for 2 weeks, and only at night for the next 2 weeks. On the fith day after surgery I don’t mind if they start to take shower. I do not need to take stitches because I use soluble material in the ear seams. I tell my patients that there may be color change, redness and bruising in the auricle for the first 2-3 days and this is normal in this process. I discharge the patients with a painkiller and an antibiotic against the pain that may occur in the first 3-4 days. I do not make any restrictions on eating or drinking.

Is Prominent Ear Genetic?
Prominent ear deformity occurs when the baby’s ear folds, which are expected to be in the auricle, are not formed during development in the mother’s womb. Having prominent ear problem in one of the parents increases the probability of the child having a prominent ear problem , so familial factors play a large role in the development of this deformity . Since prominent ear deformity can cause psychological problems in childhood, surgery is usually recommended at the beginning of school age or around the age of five-six in the pre-school period. It can be performed under general anesthesia in children and under general anesthesia or local anesthesia in adults. Another importance of having surgery at a young age is that the cartilages are soft and not fully developed yet, and they can be reshaped very easily.
When are the stitches removed in prominent ear surgery?
In prominent ear surgery, the incisions and stitches are behind the ear. These stitches can be non-melting nylon or silk stitches as well as dissolving stitches. The stitches applied to shape the ear are non-melting stitches and are not removed and are located in the auricle. If sutures are to be removed they can be taken between seventh and ten days.
Will there be a scar after the prominent ear surgery with thread?
Prominent ear surgery with a thread is usually performed on a group of patients who are children, whose cartilages have not yet hardened, and who do not have very obvious concha hypertrophy. The probability of recurrence of the pathology is higher than normal surgery, but it is a short procedure of fifteen minutes. Since there is no incision, there is no scarring.
How Is The Recovery Process After Prominent Ear Surgery?
Prominent ear surgery is performed to treat the ears to stand open to the sides when it is viewed from the front and from the back. There are usually two pathologies that cause the ears be like that . One of them is that the hollow part of the ear, which we call the concha, is relatively large, and the other is the absence of the ear fold, which we call the antihelix. Patients usually have both of these problems, but sometimes only one. Solving these two pathologies, which are also performed in surgery, is to remove a piece from the concha if there is a bigger size of concha, and to create an antihelix fold with sutures if there is no anthelix fold. The operation takes about two hours. After the procedure, edema, swelling and bruising develop in the ear , the ears are wrapped for a day to prevent blood accumulation in the ear, and the dressing can be opened after a day. It is normal leakeage of blood from the ear on the first day, sometimes even on the second day. The stitches are behind the ear , so they are not visible . After a day, the patients usually switch to a head bandage. Patients use ear bands for about two weeks. On the fifth day, I tell my patients that they can take a bath. Water or foam may touch the ear stitches. However , it should be dried after bathing and should not be left wet . In the first four-five days, patients may find their new ears very large, this is completely due to swelling and it is normal, patients are told that their ears will shrink and there is nothing to worry about. After the first week, the ear bruises pass, the swelling passes quickly, and the ear returns to its normal size. My patients, whose ear bands do not interfere with working, can usually start working on the fourth -fifth day. If it is desired to do sports, sports can be started after six-eight weeks.
Are There Any Problems After Prominent Ear Surgery?
There are two most common complications of prominent ear surgery. Hematoma and infection. A hematoma is a collection of blood between the skin and cartilage. It causes destruction of the ear cartilage when it is not noticed or not emptied. It happens when the vein that was cauterized during the operation starts to bleed again. The treatment is to evacuate the hematoma, to find the vein and to control the bleeding again. It usually happens on the first or second day of the surgery, unilateral swelling and new onset pain in the ear may suggest a hematoma. Another complication is infection. This manifests itself as unilateral ear swelling, redness, pain and fever. As a treatment, the patient’s orally administered drugs are discontinued and intravenous or intramuscular broad-spectrum antibiotics are started. Apart from these, problems such as opening in the stitches, or the auricle being closer to the head area than desired may develop.
How Many Hours Does Prominent Ear Surgery Take?
Prominent ear surgery is a procedure that takes approximately forty-five minutes to an hour on each side. This procedure can be performed under local anesthesia in adults, there is no pain during or after the procedure. An advantage of local anesthesia is that the patient can see the procedure immediately and can express his/her own opinion. However, there may be adult patients who also prefer general anesthesia. The folds that do not occur during the process are created with stitches. The incision remains behind the ear. If dissolving stitches are used, the stitches may not be removed, non-dissolving stitches should be removed between seven-ten days. Swelling and bruises in the ear after the surgery are normal and disappear rapidly. However , you should definitely consult your doctor in case of unexpected unilateral swelling , pain , redness.
Which Type of Anesthesia is Used in Prominent Ear Surgery?
Local anesthesia is generally preferred for adults in prominent ear surgery. The reason for this is to perform the procedure painlessly with local anesthetics, to communicate with the patient during the operation, to accelerate the return to social life and to reduce the cost of the operation. However ; Some patient groups who have panic attacks, who have a systemic disease such as asthma, have local anesthetic allergy and want to benefit from the comfort of general anesthesia may prefer to have this surgery under general anesthesia. In childhood, this surgery is performed in the hospital under general anesthesia. The process takes approximately an hour and forty-five minutes – two hours. It is possible to be discharged on the same day. Ear dressing remains for a day. Then the patient starts to use the headband.
How Long Does the Pain Last After Prominent Ear Surgery?
After prominent ear surgery, there is usually pain for a day, after that there is not much pain. However, this is not a rule, it may differ from person to person. Sometimes one ear may be normal while the other ear may ache. However, if there is more pain than expected, especially if pain is on one side, it may be useful to be alerted and consult your doctor. The ear is a region with a lot of vessels, and there is a lot of small bleeding during the surgery. In the surgery, these bleedings are taken under control, but sometimes one of the veins that were burned or tied after the surgery may reopen and blood may accumulate in the ear. This causes pain and swelling in the ear. This is called hematoma and it is the most common complication after prominent ear surgery, it is usually unilateral. When such a situation develops, it is necessary to drain the re-accumulated blood and to provide bleeding control. Antibiotics and painkillers are given to the patients after the operation so that the auricle of the patients does not become infected. In some cases, infection in the ear may develop rarely after the surgery, and this may manifest itself in the patient with swelling, redness, fever and pain, and the antibiotic should be changed and treated immediately.
