Eyelid Aesthetic

Although it is not as popular as rhinoplasty, we can mention that eyelid aesthetics has become a necessity for many people in recent years. Another name for eyelid aesthetics is known as blepharoplasty. It draws attention as a procedure applied to the lower and upper eyelids. It has purposes such as eliminating skin sagging and removing extra muscle and fatty tissue. It is also applied for purposes such as stretching the tissues around the eyes.

There are many factors that make eyelid aesthetics necessary. First of all, due to aging, sagging of the skin occurs with the effect of gravity. Furthermore , as a natural consequence of aging, some bags on the eyelids are encountered. Findings such as loosening of the skin, sagging of fat bags, wrinkles, discoloration; it causes people to make surgery decisions for eyelid aesthetics. In addition, deterioration occurs around the eyelids due to environmental and personal factors. Exposure to harmful sun rays, sleep disturbance, air pollution, alcohol and cigarette use; are among the factors that fasten the aging process of the skin.

    Why is Eyelid Aesthetics Needed?

In order to eliminate the negative effects of the signs of aging on the eyelid, eyelid aesthetics is necessary. The fact that the skin has an elastic structure is the reason for all these problems. Because, due to aging, the elasticity of the skin decreases. Therefore, the aging effect first appears on the eyelids. Due to the changes in the eyelids, many people have a dull, tired and old appearance on their face.

    At this point, among the symptoms that make eyelid aesthetics mandatory;

    • Color change under the eyes
    • Occurrence of bags under the eyes
    • Sagging of the eyelid and the
    • formation of some wrinkles
    • The appearance of lines called crow’s feet around the eyes
    • a tired facial expression, as we mentioned above

The problem of the upper eyelid drooping occurs due to the abundance of skin on the eyelids. This situation; in some cases, it reaches a level that prevents people from seeing. Therefore, people apply for eyelid aesthetics to treat this condition. In addition, the problem of drooping eyelids occurs due to the low eyebrows and forehead. As a result, upper eyelid or lower eyelid aesthetic surgery or both of them can be performed according to the demands of those who want to apply for this operation due to loss of function or aesthetic reasons.

      When Should Eyelid
      Aesthetics Be Performed?

Eyelid aesthetic surgery ; As expected, it is preferred by people aged 35 and over. The reason for this is that the signs of aging on the eyelids occur especially after these ages. Thanks to eyelid aesthetics, it is possible to delay the aging in this area for a maximum of seven or eight years.

What Should Be Taken Into Consideration for Eyelid Aesthetics?

As in rhinoplasty, aspirin or other antibiotic drugs should not be used at least 15 days before the procedure, due to the risk of increasing bleeding tendency. In addition to this, smoking and alcohol use should be suspended for a while.

How is Lower Eyelid Aesthetic Surgery Performed??

Lower eyelid aesthetics is the surgery to remove the prominent eye bags and remove the sagging, wrinkled excess skin under the eyes and stretch the remaining skin muscle tissue. It is done for the purpose of facial rejuvenation. An incision is made under the eye, but in some cases, this incision remains inside the eyelid.

How is Upper Eyelid Aesthetic Surgery Performed?

It is the process of removing excess skin and muscle tissue in the area. In some cases, a part of the fat tissue is also removed. It gives good results if applied together with eyebrow lifting and forehead lift operations

How long does eyelid surgery last?

Eyelid surgery can be considered as a face-lift surgery. It is the process of suspending sagging skin, subcutaneous muscle tissue and fat tissues, and removing some skin. Fat parts are sometimes removed and sometimes detransferred and used in augmentation. Therefore, it has a rejuvenation effect in patients. Aging continues from where it left off, but it creates an effect that will take the patient few years back. However, the aging process will continue. The newly formed tighter skin and subcutaneous tissue may still sag over time, but this period slows down even more. Eyelid surgery is an aesthetic procedure that can be performed under short-term local anesthesia, but with a great effect. Incision scars are hidden inside in upper eyelid crease , and under the eyelashes in the lower eyelid.

Will there be any scars after upper eyelid surgery?

The purpose of upper eyelid surgery is not just to remove the skin. If there is excess muscle and adipose tissue, muscle and fat tissue are also removed, if the eye crease is deep and above, an eye crease is created where it should be is provided. While doing these, the incision is usually in the eyelid crease if the patient’s eyelid crease is in its normal place, and if the eyelid crease is not in its normal place, it is 8-10 mm above the eyelashes in women and 6-8 mm above the eyelashes in men, and the patient’s new eyelid crease is created here. Therefore , the incision is always within the eyelid crease , and there is no visible scar from the outside , the scar is hidden . Upper eyelid surgery is a 45-minute procedure performed under local anesthesia. There is no pain during the procedure. Patients can return to their social life immediately, but it should be shared with the patient beforehand that there will be bruising on the upper eyelid in the first week.

      Is Eyelid Surgery Risky?

Of course, eyelid surgery has risks, as with any surgery. There is no life-threatening risk. These risks are different for the upper eyelid and lower eyelid. The most feared risk of upper eyelid surgery is bleeding during fat removal, but the risk is very minimal if done carefully. I have never encountered it in my own practice. Another important risk is the inability to close the eyes, which we call “lagophthalmus”, due to excess skin removal. This may be mild in the first 24 hours of the surgery and is normal, then the eye can be closed immediately. However, if it doesn’t, consult your doctor again. Other risks include insufficient skin removal, infection, and upper eyelid retraction.

The risks of lower eyelid surgery are slightly different. The thing we fear most here is that the lower eyelid will move towards the lower direction and the white part of the eye will become more visible. We call this “retraction” or “ scleral show “ . Sometimes, it can be severe enough to turn the lower eyelid outward and it can be an event called ectropion. There are various reasons for this retraction , such as excess skin removal , adhesions in deep tissues , and excessive removal of muscle tissue . Another risk is the damage to the muscle that makes the eye look downward, which is very rare and not very common in practice. When it occurs, problems such as double vision and strabismus occur. We can count other risks such as infection, bleeding, hematoma (blood accumulation).

What is the Age Range for Eyelid Aesthetic Surgery?

There is no age limit for eyelid surgery. However, it is not a common surgical method at early ages. Such problems are not encountered at an early age, as it is usually done due to age-related sagging of the upper eyelid or sagging of the lower eyelid and eye bags. In middle age, the problems are still mild and patients usually try to solve their problems with non-surgical methods. However, eyelid surgery is rarely performed at younger ages for patients with eyelid asymmetry or significant eyelid muscle hypertrophy due to genetic or structural reasons.

Is a Bath Taken After Eyelid Aesthetics?

It is not a problem if water touches the incisions on the eyelid after eyelid aesthetic surgery. I have almost never encountered an infection in this area. We also give patients an antibiotic treatment on discharge for prophylactically for a few days. Therefore, taking a bath after surgery is not a problem. However, I still recommend our patients to wash the areas other than the eye area, if possible, for the first 5 days. If water or foam has touched the eye, there is no need to worry. In the first 48 hours, the eye area will swell anyway, this has nothing to do with water contact, it should not be considered as an infection.

How Long After Eyelid Surgery Are Lenses Used?

Patients cannot use contact lenses for 2 weeks after eyelid surgery. In this process, patients are recommended to use glasses. After 2 weeks, it is recommended that patients start wearing contact lenses using the lower eyelid without forcing upper eyelid.

How Many Hours Does Eyelid Surgery Take?

The duration of eyelid surgery varies. The duration of a standard upper eyelid surgery is approximately 45 minutes. However, if fat injection and ptosis (low eyelid) surgery are to be added to this, this period may take up to 2 hours. A standard lower eyelid surgery is a procedure that takes approximately 1.5 hours.

How Many Days Does Eyelid Aesthetics Heal?

Eyelid aesthetic surgery is usually performed under local anesthesia and sometimes under general anesthesia. Patients do not stay in the hospital after the operation, and they can return to their homes on the same day. In upper eyelid surgery, they are discharged from the hospital or clinic with dressing made with bands covering the upper eyelids. They are discharged with a dressing made with a band that covers the incision mark on the lower eyelid surgery. In the first 2 days, edema and bruises occur around the eyes. It is very normal to have blood leakage from the dressing on the first day. Patients are advised not to drink blood thinners, avoid herbal teas, and not use herbal products 1 week before and 3 days after the surgery. These products increase bleeding during and after surgery. It is recommended that they control their blood pressure, and if they are using blood pressure medications, they should use these drugs regularly. After 1 day, the bands on the eyes can be removed, a bath can be made. Water and foam touching the incision is not a problem. Post-operative pain is mostly absent, but I give all my patients an antibiotic to use for a few days during discharge and a pain killer to use when there is pain. On the 3rd day, the swelling is at its maximum size and now begins to decrease, it goes down to the lower parts of the face. It is not recommended for patients to drive for the first 3 days. If they are using contact lenses, they can start using lenses again after 14 days.At the end of the first week, the swelling has decreased, the bruises around the eyes have turned yellow, it can be camouflaged with make-up and return to social / business life. After 2 weeks, only a redness remains at the incision site. After 3 months, the incision scar becomes almost invisible, most of the scar is already hidden in the eye crease.
